INFORMATION TEXT PURSUANT TO THE LAW ON THE PROTECTION OF PERSONAL DATA

As regulated in Article 1 of the Law No. 6698 on the Protection of Personal Data; The purpose of this Law is to protect the fundamental rights and freedoms of individuals, especially the privacy of private life in the processing of personal data, and to regulate the obligations of real and legal persons who process personal data and the procedures and principles to be followed.
Your personal data may be processed by Furkan Ofset, as Data Controller within the scope of the Law No. 6698 on the Protection of Personal Data and the relevant legislation, within the framework described below and in accordance with the Basic Law on Health Services No. 3359, the Decree Law No. 663 on the Organization and Duties of the Ministry of Health and its Affiliated Organizations, the Regulation on the Processing of Personal Health Data and Ensuring Privacy, the Regulation on Private Hospitals, the Regulation of the Ministry of Health and other legislation.
This Information on the Protection of Personal Data explains our personal data protection and processing policy and how we collect, transfer, use and protect your personal data during the services performed by our hospital.

1. ACQUISITION, PROCESSING AND METHOD OF PERSONAL DATA
Your personal data is collected by Furkan Ofset for the purposes of protecting public health, preventive medicine, records related to your applications to our health institutions, medical diagnosis, treatment and care services, planning and management of health services and financing, and in accordance with the fields of activity of our company; verbal, written, visual or electronic media, online through the SSI system, from the records shared in case of benefiting from a private insurance company, through the records of other health institutions in case you come to our health institution by referral, through the e-mails you send, call center, website, mobile applications, online services, verbal, written and similar means. Your personal data of special nature, especially your health data, and your personal data of general nature can be processed by Furkan Ofset in a measured and measured manner in connection with the purposes specified in this article, including but not limited to the following:
1. Your identity information: Your name, surname, Turkish ID number, passport number or temporary Turkish ID number, place and date of birth, marital status, gender, insurance and/or patient protocol number, health insurance, occupation, insurance card number, workplace registration and/or patient identification number and other identification data that can identify you;
2. Your Contact Information: Your address, telephone number, e-mail address and other contact data, your voice call records kept by customer representatives or patient services in accordance with call center standards, and your personal data obtained when you contact us via e-mail, letter or other means;
3. Your financial data such as your bank account number, IBAN number, credit card information, billing and invoice information;
4. Your data regarding private health insurance for the purpose of financing and planning of health services and your Social Security Institution data,
5. All kinds of general and private health data obtained during or as a result of the execution of medical diagnosis, treatment and care services, including but not limited to patient medical reports, diagnostic data, biometric and genetic data, laboratory results, test results, examination data, check-up information, doctor analysis and comments, appointment information, prescription information,
6. Your notifications such as questionnaires filled out by patients, letters of thanks and complaints, satisfaction results,
7. Your vehicle license plate information in case you use the parking lot / if your companion uses it and if you use the valet service,
8. Your images obtained from camera recordings that are continuously recorded in common areas in our hospital in accordance with the legislation,
9. Your voice recording of your communication with our hospital,
10. Your identity information collected during your visit to www.furkanofset.com.tr and your health data, IP address and other personal data that you submit and/or enter through online services,
11. In case you apply for a job, your other personal data, including your resume provided in this regard, and all kinds of personal data related to your service contract.

2. PURPOSES OF PROCESSING YOUR PERSONAL DATA
In accordance with the provisions of Law No. 6698, including but not limited to the following, all kinds of personal data of private and general nature obtained by Furkan Ofset; It can be processed for the following purposes:
1. Identification and confirming your identity,
2. Protection of public health, preventive medicine, medical diagnosis, treatment and care services, planning and management of health services and financing,
3. Planning and management of Furkan Ofset internal functioning,
4. Supply of medicines or medical supplies, tools, equipment and devices,
5. If you make an appointment, informing you about the appointment, providing information, reminding you about the appointment,
6. Fulfillment of risk management and quality improvement activities,
7. Fulfillment of legal and regulatory requirements,
8. Verification of your relationship with the institutions contracted with Furkan Ofset and financial reconciliation with these institutions regarding the health services provided to you,
9. Sharing and authenticating information requested by private insurance companies, contracted institutions and organizations within the scope of financing health services,
10. Billing for our services,
11. Sharing and responding to information requested by the Ministry of Health and relevant public institutions and organizations in accordance with the relevant legislation,
12. Taking all necessary technical and administrative measures within the scope of data security of Furkan Ofset systems and applications,
13. Analyze your use of health services and store your health data in order to develop and improve the health services we provide to you, and respond to your questions or complaints regarding our services,
14. Providing the necessary information in line with the requests and audits of regulatory and supervisory bodies and official authorities,
15. Maintaining information about your health data that must be kept in accordance with the relevant legislation,
16. Measuring patient satisfaction and increasing patient satisfaction after receiving health services,
17. Training and developing our employees, monitoring and preventing fraud and unauthorized transactions,

18. Newborn baby notification, inpatient, companion, vehicle license plate number notifications when necessary,
19. To fulfill our legal obligations under the Basic Law on Health Services No. 3359, Decree Law No. 663 on the Organization and Duties of the Ministry of Health and its Affiliated Organizations, Regulation on Private Hospitals, Law No. 6698 on the Protection of Personal Data, Regulation on the Processing of Personal Health Data and Ensuring Privacy and other relevant legislation,
20. Carrying out promotional and informational activities and ensuring that you benefit, contacting you for information purposes regarding our services,
21. Compliance with internal policies and principles,
22. Without being limited to these, the execution, development of medical diagnosis, treatment and care services, planning and management of health services and financing, increasing patient satisfaction, research and similar purposes.
During the processing of data, no violation of your personal rights will be allowed. However, your personal data will not be used for any commercial purpose except for the activities listed above and as required by the relevant legislation.

Your personal data obtained and processed in accordance with the relevant legislation can be transferred to Furkan Ofset’s physical archives and / or information systems and kept both in digital and physical environment and can be shared only at the request of official institutions (Courts, Attorney General’s Offices, Ministries, etc.) when permitted by the legislation.
A copy of your health data such as examination results and reports that you have submitted to our hospital regarding processes such as examinations and treatment in other health institutions will be kept in your inpatient file, and your health data such as X-rays will be destroyed within three months if you do not receive them when you are discharged.

3. TRANSFER OF PERSONAL DATA
By ensuring that all necessary technical and administrative measures are taken by Furkan Ofset to ensure the appropriate level of security in accordance with the KVKK and the relevant health legislation, your personal data, the Basic Law No. 3359 on Health Services, the Decree Law No. 663 on the Organization and Duties of the Ministry of Health and its Affiliated Organizations, the Regulation on Private Hospitals, the Law No. 6698 on the Protection of Personal Data, the Regulation on the Processing of Personal Health Data and Ensuring Privacy and the persons / institutions and / or organizations permitted by the provisions of other relevant legislation, private insurance companies, the Ministry of Health and its related units, the Social Security Institution, Family Medicine Centers, the General Directorate of Security and other law enforcement agencies, the General Directorate of Population, the Pharmacists Association of Turkey, banks, funds, foundations, laboratories in Turkey or abroad that we cooperate with for medical diagnosis, medical centers and third parties providing health services, the health institution to which the patient was referred or to which the patient applied, the institution to which you are affiliated or employed, courts and all kinds of judicial authorities, centers and other third parties, Your authorized representatives, lawyers, third parties from whom we receive consultancy including tax and financial advisors, regulatory and supervisory bodies, business partners and other third parties with whom we cooperate to develop or carry out health services for the above-mentioned purposes, including government authorities.

4. METHOD AND LEGAL GROUNDS FOR OBTAINING PERSONAL DATA
Your personal data may be collected in all kinds of verbal, written, visual or electronic media, telephone, sms, mms, etc. Your personal data is obtained through telecommunication communication means, online through the Social Security Institution system, from the records shared in case of benefiting from a private insurance company, in case you come to our health institutions with a referral and / or if you transmit it yourself, through the records of other health institutions and organizations, through the mails (e-mails) you send, call center call records, website, verbal, written, printed and similar channels such as magazine subscription form, for the above-mentioned purposes and in order to provide health services within the legal framework determined and in this context, Furkan Ofset can fully and duly fulfill its contractual and legal obligations.
When the purpose requiring the processing of your Personal Data in accordance with Article 7/1 of the KVKK disappears and/or the statute of limitations / retention periods that we are required to process your data in accordance with the legislation expire, your Personal Data / Personal Data will be deleted, destroyed or anonymized.

5. YOUR RIGHTS REGARDING THE PROTECTION OF PERSONAL DATA
As a personal data owner, in accordance with the legislation regarding your personal data processed;
1. To learn whether any personal data is processed,

2. To access and request personal health data;
3. To learn the purpose of processing personal data and whether they are used in accordance with their purpose,
4. To know the third parties to whom personal data are transferred domestically or abroad,
5. To request correction of personal data in case of incomplete or incorrect processing,
6. To request the deletion or destruction of personal data other than the information necessary for Furkan Ofset to fully and properly fulfill its contractual and legal obligations,
7. In case of incomplete or incorrect processing of personal data, to request that third parties to whom personal data are transferred be notified of the transactions regarding the correction and/or deletion or destruction of personal data,
8. To object to the emergence of a result to the detriment of the person himself/herself by analyzing the processed data exclusively through automated systems,
9. We hereby inform you that you have the right to demand from us the compensation of the damage in case of damage due to unlawful processing of personal data.

If you exercise your rights listed above, the relevant information will be notified to you in a clear and understandable manner in writing or electronically through the contact information provided by you.

6. DATA SECURITY
Furkan Ofset attaches importance to the protection and security of your personal data, protects it in full compliance with all technical and administrative security controls that must be taken in accordance with information security standards and procedures and at an appropriate level against possible risks, acts with the awareness that personal data security is at the forefront, and shows all necessary sensitivity in this regard.

7. CASES WHERE PERSONAL DATA MAY BE PROCESSED WITHOUT EXPLICIT CONSENT IN ACCORDANCE WITH THE LAW ON THE PROTECTION OF PERSONAL DATA
Pursuant to Article 5/2 of the Law on the Protection of Personal Data, it is possible to process your personal data specified below without seeking your explicit consent in the following cases.
a) It is explicitly stipulated in the laws
b) It is mandatory for the protection of the life or physical integrity of the person who is unable to disclose his consent due to actual impossibility or whose consent is not legally valid, or of another person
c) Provided that it is directly related to the conclusion or performance of a contract, it is necessary to process personal data of the parties to the contract
ç) It is mandatory for the data controller to fulfill its legal obligation
d) It has been made public by the person concerned
e) Data processing is mandatory for the establishment, exercise or protection of a right
f) Data processing is mandatory for the legitimate interests of the data controller, provided that it does not harm the fundamental rights and freedoms of the data subject.

8. COMPLAINTS AND COMMUNICATION
